What Are Your Thoughts on Self Care?

Richmond AID is carrying out a consultation with NHS Richmond which is about looking for peoples’ thoughts on ‘self care’ – about managing your long-term health condition.

The aim of the consultation is to find out what information and resources are needed for people to manage their health condition better.

If you have a long-term health condition, and have a bit of time to spare, you can have a look at our questionnaire below. If you are interested in taking part in this survey, NHS Richmond is very keen to hear your views on this.

If you can print the form, you could complete it and post or drop it off to us at the Disability Action and Advice Centre, 4 Waldegrave Road, Teddington, TW11 8HT.
